Because of our amazing turnout last week for our first pre-enrollment meeting, we expect a phenomenal turnout for our first enrollment opportunity. Therefore, to prevent extensive lines and make the enrollment process as smooth as possible, we are requesting parents attend enrollment sessions based on alphabetical order by last name:

• Friday, February 19, 5:00 pm – 9:00 pm (A-E),
• Saturday, February 20, 2:00 pm – 5:00 pm (F-J)
• Friday, February 26, 5:00 pm - 9:00 pm (K - P),
• Tuesday, March 9, 5:00pm – 7:00 pm (Q - S)
• Thursday, March 11, 5:00 pm - 7:00 pm (T - V),
• Friday, March 19, 5:00pm – 9:00 pm (W - Z)

IF YOU ARE UNABLE TO ATTEND YOUR DESIGNATED SESSION, YOU MAY ATTEND
ANY OF THE SESSIONS LISTED.

Incoming Kindergarten Students: Incoming kindergarten students must complete all forms except for the "State Health Exam Form."

Incoming 1st Graders: The "State Health Exam Form" must be completed by incoming first grade students only, prior to the beginning of the school year. It does not have to be completed in order to enroll your child in first grade. The form mst be completed by a physician. Incoming first grade students must complete all other forms EXCEPT the "Kindergarten Enrollment Form."

Students in Grades 2 - 8: All incoming students to grades 2 - 8 must complete all attached forms EXCEPT the "State Health Exam Form" and the "Kindergarten Enrollment Form."

You are encouraged to download, print and complete these forms (as indicated above) prior to coming to OPA's Enrollment Center, located at Royalty Cakes at 5236 D Street, Chino, CA 91710.

Enrollment will take place,

• Friday, February 19, 5:00 pm – 9:00 pm
• Saturday, February 20, 2:00 pm – 5:00 pm
• Friday, February 26, 5:00 pm - 9:00 pm
• Tuesday, March 9, 5:00 pm – 7:00 pm
• Thursday, March 11, 5:00 pm - 7:00 pm
• Friday, March 19, 5:00 pm – 9:00 pm

To complete a student admission application, you must bring the following documents:

• Proof of residency (i.e. utility bill)
• Proof of immunizations
• Proof of age (i.e. birth certificate, passport)
• Copy of student's IEP or 504 plan, if applicable
• Parent signature on Learning Partnership Agreement
• Completed Home Language Survey
• Most recent grade report (A copy of a recent report card will help ensure the academic needs are met for your child)
  

Please remember... you must attend ONE (1) Pre-Enrollment Information Meeting PRIOR to enrolling your child in OPA.
